Individual consultations are available in person or via telehealth or phone on a secured health platform. I accept the following insurance plans: Blue Cross Blue Shield, Medicare, Cigna, and United Health.

Insured Clients

Most insurance plans consider nutrition counseling to be a preventative service, covered at 100% with no patient responsibility! Please verify your specific coverage plan!

Some insurance plans consider it a medical service. Most insurance plans cover nutrition appointments via telehealth the same as in-office. Visit might be covered as follows:

  • Covered at 100% with no patient responsibility
  • Payment subject to specialist copay, deductible and/or coinsurance

Self Pay Clients

A valid credit/debit card on file is required and will be used for self-pay fees, any copayments, deductibles, and non-covered services such as meal plans. You may also keep an HSA/FSA card on file for eligible transactions.

Self-pay rates are as follows: 

$ 250 for your initial visit (60 minutes)

$450 package for 3 follow up visits (45 minutes)

…you may be able to see a dietitian at zero cost to you!

Important Information for Medicare Patients

  • Visits are covered ONLY for patients with diabetes, kidney disease (CKD, but pre-dialysis), or 36 months post-kidney transplant.
  • You need a referral from your doctor before scheduling your visit. A new referral form MUST be obtained for each calendar year. Call Your doctor BEFORE your visit and ask for the referral to be faxed to me at 844 638 5296.
  • The referral should include your diagnosis code (ICD-10), Physician full name and Physician NPI.
  • Medicare does not cover pre-diabetes, weight loss, high cholesterol, or other diagnoses.
  • Medicare covers 3 hours (2-3 visits) for the first calendar year and 2 hours (1-2 visits) the following years.
  • Additional, unlimited visits are covered with the second referral, when there is a change in your diagnosis. Ask your physician to fax to me at 844 638 5296.

Our Policies & Procedures

  • Please complete all forms within 24 hours of your appointment to avoid cancellation.
  • Please download labs and referral to documents in your secured portal.
  • If you schedule an appointment for your child, include your child’s name, not your own, on all forms.
  • All payments are expected at the time of visit, including out-of-pocket, co-pay, and coinsurance. Please bring an insurance card, photo ID, and necessary doctor referrals.
  • A credit card on file is required.
  • BLS Nutrition requires 24 hour advance notification to cancel an appointment. You are responsible to pay for appointments that are not canceled within 24 hours of your scheduled visit.
  • Late Cancellation & No Show Fees:  $100
